Vaccination Clinics
Tuesday, October 5th and Thursday, October 7th at the schools.
In order for Public Health nurses to administer
vaccines, the consent form (LINK) and insurance information must
be completed, signed and returned to your student’s school by
Monday, September 27, 2021.
Note: Vaccines will not be given to any student without completed
paperwork and insurance information. Moreover, if you do not want
your student to receive any vaccinations, please do not return the
consent form. Only students with properly completed consent forms
can participate.
Insurance: Please attach a copy of your insurance/ Medicaid card (both
sides) to the consent form.
If you have no insurance, please call the ADLC Public Health
Department for assistance in obtaining any needed vaccines (your child
may qualify for free immunizations).
